Can the bank add something to ur acc without permission?

we had a bank add upps prtection to our acc without us saying yes and myn wife turned it down when she opened the acc. now that happened it sent us into over draft an has charged us alot of money|||So go see the bank. Are you sure you didn't sign something or fail to "opt out" of it?
